The Power of Impossible Thinking (by Yoram Wind and Colin Crook, 2006 ) talks of why it is so hard to change the way we think and offers some advice on how to overcome the inhibitors to change.

A recommended reading for MBA students, even if you are not too keen on neuroscience;)
